2008 SODA Terms of Use Insurance Agreement
Submission Does Not Guarantee Coverage
I understand and agree that the submission of this online insurance enrollment does not guarantee coverage even though the enrollee may immediately receive an email verifying that the order was received. The online insurance enrollment must be approved by Sadler & Company, Inc., prior to binding of coverage. In the event that Sadler & Company, Inc determines that the enrollee does not qualify for the program, has not accurately answered a question on this online enrollment, has not provided truthful information during past transactions, or if the credit card transaction is or has been rejected or reversed on the current online enrollment or during past transactions, coverage will be declined and the enrollee will be notified normally within six business days.
Coverage Effective and Expiration Dates
I understand and agree that coverage starts the later of 1-1-2008 or the date that this fully completed and accurate enrollment form is received at SODA headquarters and approved by Sadler & Company, Inc., concurrent with the payment of the total amount due by credit card. Coverage Period From 1-1-2008 to 12-31-2008. All coverages for teams & leagues expire 12-31-2008 regardless of effective date. Coverage for Tournaments & Camps & Clinics will expire on the last date of the event listed on the enrollment form.
Insurance Details
I agree that I have read and understand the View Insurance Program Details section of the SODA website at www.sportsplexoperators.com/insurancemain.asp which provides a description of coverages for Accident and General Liability, risk management requirements, and the activities and operations for which coverage is not provided.
You Can Request Complete Copy of Policies
I understand and agree that I must refer to the Accident and General Liability policies for complete information on policy coverages, conditions, limits, and exclusions and that the Insurance Details are merely a summary for illustrative purposes. I understand that I can request a complete copy of the policies by sending an email to soda@sadlersports.com.
Fraud Warning
I understand that the insurance company, in determining whether to provide insurance coverage will rely on the information contained in this online enrollment and all other information being submitted. I hereby warrant, represent and confirm that, to the best of my knowledge, all information provided is complete, true and correct.
I am aware that the insurance company expects accurate reporting for my premium calculation, and should my figures exceed my estimates during the coverage term I will make arrangements to pay the additional premium. I understand that my books and records may be examined or audited by the insurance company at any time during the coverage period and up to three years afterwards. Intentional misrepresentation or misreporting may jeopardize coverage.
Compensation Disclosure
I understand and agree by my electronic signature below that SODA will receive a fee in exchange for affiliation and certain services rendered. In addition, Sadler & Company, Inc., in consideration of insurance and risk management services rendered, will receive a fee in lieu of commission on the General Liability policy (equal to 20% of premium). Risk Purchasing Group
I understand and agree that the completion of this enrollment form confirms our desire to obtain General Liability insurance through the ERS Risk Purchasing Group Association, Inc., domiciled in IL, if such coverage is chosen.
NOTE: CHARGES ARE FULLY EARNED AT INCEPTION AND THERE ARE NO PROVISIONS FOR REFUNDS
